1
Hello, my name's Jade, I'm one of the doctors.
2
Hi.
3
Could I just take your name and date of birth, please?
4
Yeah, it's Eve Pollack.
5
It's the second or the third, 1990.
6
Okay, that's great.
7
Thanks, Eve.
8
What's brought you in today?
9
I've been suffering from really severe headaches recently.
10
Okay, could you tell me a bit more about the headaches?
11
Yeah, just very painful, really debilitating to be honest
12
and I'm getting a bit worried about them now so I just wanted to come and have a chat about it.
13
Yeah, understandable.
14
So how long have these headaches been going on for?
15
It's been about six months in total now.
16
Okay, so quite a while.
17
And whereabouts do you feel the headaches?
18
It's usually just on one side of my head.
19
It can be left or right but it's just on that one side.
20
Okay, and what does it feel like?
21
I describe it as sort of like a really kind of deep sort of throbbing,
22
you know, like a pounding type of headache.
23
Yeah, so when the headache comes on,
24
is it all of a sudden or does it gradually build up?
25
It sort of gradually comes on
26
and then I'd say over a period of sort of minutes it kind of gets worse and until it,
27
yeah, it's very painful and then it kind of stays like that for the rest of the time.
28
How long does the headache tend to last?
29
Oh, hours.
30
It can be all day,
31
all night to the point where I just have to try and get myself off to sleep to make it go away.
32
Okay.
33
And does the pain move anywhere else?
34
No, it just stays on the one side.
35
Okay.
36
And do you ever get any other symptoms alongside the headache?
37
Yeah, when I've got the headache,
38
I tend to feel quite sick with it.
39
Okay.
40
Have you ever been sick with it?
41
No, I've never been sick.
42
I just feel really nauseous.
43
Have you ever experienced any sort of stiffness in your neck with the headache?
44
No. Okay.
45
And is there any particular times you've noticed the headache coming on?
46
I've noticed a couple of times it's been when I've been particularly tired from work or maybe a little bit stressed.
47
And there's also been a couple of times where it's sort of happened at the start of my period.
48
Okay, how frequently do you think they're happening?
49
The headaches are probably once or twice a month.
50
Once or twice a month, okay.
51
Any particular time of day it tends to occur?
52
Probably mostly afternoons, I would say,
53
maybe when I finish work,
54
yeah, that sort of thing.
55
Okay, and has it ever woken you up in the night or in the morning?
56
No. Okay, and then, so you said sometimes at the start of your period,
57
how long is your cycle usually?
58
28 days.
59
28 days, okay.
60
And are you on any contraception?
61
Yes, I take the mini pill.
62
The mini pill, okay.
63
And anything you've noticed that makes headaches worse?
64
Yeah, kind of bright lights,
65
loud noises, that sort of thing that just makes it more uncomfortable.
66
And in terms of other things that might make it worse,
67
you haven't noticed it's affected by standing up or laying down?
68
No, it tends to just not make a difference, that sort of thing.
69
Okay, anything that makes it better?
70
No, not really.
71
I tend to just have to go and lie down in a quiet,
72
dark room and just wait for it to pass.
73
Have you tried any medications at all?
74
Just paracetamol and ibuprofen, but I don't really seem to do much.
75
How often do you tend to take them?
76
When the headache comes on I'll just take,
77
you know, maybe try a couple of paracetamol and then a couple of ibuprofen later but as I said,
78
neither of them seem to work so I've not really taken many.
79
Okay and
80
if you had to put on a scale of kind of
81
0 to 10 where 0 is no pain at all and 10 is the worst pain you've ever felt,
82
what would you rate the headaches?
83
Probably 8 out of 10.
84
I mean at times I'd go so far as to say it was 10 out of 10.
85
It's really awful.
86
Yeah, it sounds like they're really really painful.
87
How has that been impacting your life?
88
It's just awful in terms of that's sometimes I'm having to wait all day
89
and all night for it to go away
90
and that's not what I want to be doing with my days and yeah it's not pleasant it's really debilitating.
91
Yeah I can imagine.
92
And then in terms of before the headache do you ever get any symptoms before the headache comes on?
93
Kind of the day before
94
I've noticed that I feel really tired and can get a little bit irritable
95
and I feel like I just spend the day just yawning the day before I get a headache.
96
Okay and how about immediately before the headache are there any sort of warning signs that's about to come on?
97
Yeah I tend to see
98
what I can just describe as kind of like flashing or flickering lights and then just before
99
I'll get these zigzaggy kind of lines and then I know you know that's it then the headache comes on after that.
100
Okay and then you mentioned that with the headache you get a bit of nausea as well.
101
Yeah.
102
After the headache's passed do you feel kind of back to normal or do you still feel a bit...
103
No I tend to feel really washed out after the headache
104
really like sort of next day sometimes yeah the next day
105
or two I feel really tired yeah quite drained you know it really takes it out of us.
106
Okay.
107
And have you ever had any fevers?
108
No. No. Any changes to your appetite?
109
No. Or any changes to your weight?
110
No. No. Or have you ever woken up feeling very sweaty at night?
111
No. No. Okay.
112
And is there anything in particular you thought might be going on?
113
I don't know.
114
I've never experienced anything like this before.
115
So I'm just a bit baffled as to why I'm suddenly suffering from these headaches.
116
Yeah.
117
And is there anything about it that's really been worrying you?
118
Yeah, I am a little bit concerned now because with it being going on for the last six months,
119
I'm a little bit concerned that this could be something maybe a bit more sinister.
120
and maybe something like a brain tumour.
121
Okay, so that sounds like it's been really worrying.
122
Yeah.
123
Is there anything in particular you were hoping we'd be able to do for you today?
124
Just try and find out what's going on really and ideally maybe get some strong painkillers,
125
something that will just make the pain go away.
126
Yeah, okay.
127
I'm just going to run through a few more questions just about your general health.
128
Have you got any other medical conditions at all?
129
Yeah, I've been diagnosed in the past with depression and anxiety.
130
Okay, and how has your mood been with all this going on?
131
That's under control.
132
I take medication, so I feel like I've been okay.
133
Okay, yeah, that's good to hear.
134
What medication is it that you're taking?
135
Sertraline.
136
Sertraline, and how long have you been taking that for?
137
Oh, that's probably been a few years now,
138
to be honest, yeah, but I'd say that that seems to be working the whole thing here.
139
Okay, good.
140
So you're on sertraline and then you mentioned the mini pill as well,
141
and as well as the paracetamol and ibuprofen when you get the headache.
142
Yeah.
143
Is there any other medications you take?
144
No, that's it.
145
No?
146
And any allergies at all?
147
No. No. Okay, lovely.
148
And any conditions that run in the family at all?
149
My mum did mention to me that she's suffered from bad headaches in the past.
150
Yeah, so she has, she did mention that and she's actually had breast cancer a few years back.
151
She's all fine in everything now but yeah,
152
that's in terms of family.
153
Yeah, okay, glad to hear that she's better now.
154
Thank you.
155
Did she ever have any investigations for her headaches?
156
I don't think so, she never mentioned,
157
she just said that she used to get really bad headaches.
158
Okay, and I'll just ask a bit about your lifestyle now as well now as well
159
so are you working at all yes I'm a dental nurse okay
160
and
161
and has these headaches had any impact on your work I
162
have had to kind of call in get someone to come me last minute
163
which isn't again it's not ideal not something that I want to continue
164
and so yeah that's a bit of a concern okay
165
and are you living with anyone at the moment my boyfriend boyfriend okay
166
and do you smoke no
167
and do you drink any alcohol yeah i have a drink on a weekend okay
168
and what is it
169
that you tend to drink um i have a few glasses of wine on a friday
170
and saturday night okay
171
so in in a week how many glasses of wine would you have usually um say six six yeah
172
and what size are the glasses um just like a small kind of standard wine glass i guess okay
173
so usually about six standard glasses of wine in one week.
174
And just a question we have to ask everybody,
175
have you ever taken any recreational drugs?
176
No. No, okay lovely, thank you.
177
So what I'll do is I'm just gonna kind of summarise what you've told me
178
so far and if there's anything else that you think is relevant
179
or you want to add in then just let me know okay.
180
So the main problem you've come in with today is these headaches
181
which have been going on for the past six months or
182
so and they tend to be on one side of your head
183
but it can be either the left side or the right side.
184
It's kind of a deep throbbing pain but it doesn't move anywhere
185
and you often get a bit of nausea with it as well but you haven't been sick.
186
They tend to occur a couple of times a month
187
and you've noticed it may be related to when you've not slept properly,
188
when you've got a lot of stress or potentially at the beginning of your menstrual cycle as well.
189
In terms of things that make it better you've tried kind of paracetamol ibuprofen
190
but they don't tend to help you just have to go lie down in a room in the dark
191
and things that make it worse,
192
so bright lights and loud noises as well.
193
And it's been kind of usually an 8 out of 10,
194
but at its worst, a 10 out of 10,
195
which has meant it's had quite a significant impact on your life.
196
It is debilitating and impacted you at work as well.
197
And then your main worry was about whether this could be something more sinister.
198
So you're just hoping to find out what the headaches could be caused by
199
if there's any extra medications we could give you.
200
Is that right?
201
Yeah, that's right.
202
Is there anything else you'd like to add?
203
No. No, and is there any other questions that you've got for me today?
204
No, I don't think so.
205
Okay, well thank you very much for speaking to me.
206
Thank you.
207
Today I spoke to Eve,
208
a 34 year old lady presenting with headaches.
209
Eve described a unilateral headache with gradual onset building up over a few minutes that can last hours or even days.
210
Eve described the day before the headache feeling tired and irritable with excessive yawning.
211
Immediately prior to the headache she described some visual disturbances.
212
The headache itself could last hours and is associated with some nausea but no vomiting.
213
These headaches typically occur one to two times a month.
214
The timing of the day can vary but are typically in the afternoon or evening.
215
Eve has also noticed that the headaches can be associated with the beginning of her menstrual cycle.
216
The headaches can also be made worse by bright lights and loud noises.
217
She has tried using medications such as paracetamol and ibuprofen,
218
but has not found any benefit from this.
219
Eve described the pain as debilitating,
220
often an 8 out of 10,
221
but sometimes 10 out of 10.
222
This has had a significant impact on her life and she has occasionally had to miss work as a result.
223
Eve has a past medical history of depression,
224
which is well controlled using sertraline.
225
Her only other regular medication is the progesterone only pill and she takes over-the-counter paracetamol and ibuprofen occasionally.
226
Eve does not smoke.
227
She drinks six small glasses of wine a week.
228
She does not take any recreational drugs.
229
Eve currently works as a dental nurse and lives with her boyfriend.
230
Eve is unclear why she has been getting these headaches as she has never suffered from them before
231
and she is worried that there may be something sinister underlying it such as a brain tumour.
232
She would like to find out what the cause of the headaches are
233
and see if there are any additional painkillers that may be beneficial.
234
To complete my assessment I would perform a full neurological and cardiovascular examination and consider further investigations including a headache diary.

  • “Could you tell me a bit more about...?”
  • “It sort of gradually comes on...”
  • “Does the pain move anywhere else?” -
  • “I've been suffering from...”
  • “To the point where...”
