Hello this is Trina welcome to Spalli Law and today we are interviewing Isabel and we're going to talk about acne, oily skin and acne scarring.
2
So Isabel tell us 12 years old
3
so there's a lot of scarring as well okay um seven picking okay
4
and scarring and picking scarring and picking
5
and I have oily skin okay as far as I can see it's an oily skin okay
6
and anything else that is a concern of yours Not at the moment.
7
Just mostly acne, scarring, picking, and oily skin.
8
Okay, so what do you do right now to help with that?
9
Like my routine?
10
Yeah, like what is your skincare routine?
11
I cleanse So, like I guess a gentle cleanser.
12
Okay.
13
And then toner and then moisturize.
14
Okay, so you tone and then moisturize.
15
And is it tone and moisturize both morning and night?
16
Yes.
17
Okay.
18
And what kind of toner are you using?
19
I'm using like a rose water.
20
Rose water.
21
Okay.
22
And then are you doing any kind of exfoliation at all?
23
Um, I have a scrub that I use once a week.
24
Okay and what's inside the scrub is it made with like do you know what it's is it jojoba beads,
25
apricot, I don't know.
26
It's one of those like drugstore just little scrubby ones.
27
Like a fine scrub?
28
I don't know what the little balls are made of.
29
Okay.
30
Yeah.
31
So there's no like glycolic or salicylic acid or anything like that in there?
32
Salicylic, yes.
33
Salicylic, okay.
34
Salicylic and in the scrub.
35
Okay. that you're using?
36
No. Okay.
37
And any antioxidants or oils or anything like that?
38
No. Okay.
39
All right.
40
So, in this routine that you're doing right now to combat your acne,
41
you're not really using anything that's going to help stop the acne.
42
So, doing a scrub once a week isn't really going to do anything because acne's inside of the skin.
43
and it's under the skin.
44
So what a scrub does is it feels good on the surface, but if you think of like, like if there's a clog inside of a hole.
45
you're scrubbing at the surface you're not getting what's inside the hole right
46
so you have to get something that's going to go down
47
and unclog that pore so that it'll come out so
48
so scrubbing at the surface and salicylic acid is good for acne
49
but it's good for like surface acne
50
when you have already a pimple that's there
51
and it's gonna help to fight that but you want to unclog the pores
52
so that you don't get a buildup it which causes the acne
53
so stuff like that would be do ever get cystic acne
54
or is it mostly like blackheads
55
and pitbulls cystic as well where do you get those I get them around my chin sometimes my forehead okay
56
so does that usually come during your period yes okay so
57
so to help with cystic acne is one ingredient it's called benzoyl peroxide
58
and benzoyl peroxide is like a liquid oxygen and cysts are like little hard balls under is P bacteria and bacteria,
59
the P bacteria doesn't like to live in oxygen, so that's why it's contained in this circle.
60
You can't get a scrub on there and you can't get anything in there.
61
So what you want to do is use benzoyl peroxide, which can go in and put like it's like a liquid oxygen.
62
It can go in there and kill the bacteria in the pore, I mean in that sack and then the sack will go down, the bacteria will die and it'll go down.
63
You never want
64
your cysts because those when you pick them that's
65
when you get scarring because also when you pick a cyst it's like a sack
66
and if you're pushing on it and you break the sack
67
and it opens inwardly it'll cause your skin to fall in and cause like the you know the scar but also
68
it'll cause that P bacteria to escape and go to other parts around there and then you get pimples around the area.
69
So like you never want to pick a cyst.
70
You always just put benzoyl peroxide on it.
71
You can get over the counter benzoyl peroxide.
72
You can also get it from your doctor.
73
And then so that you would put that on when the cyst
74
when you very first start to feel one of those bumps coming on.
75
Usually they start to throb.
76
You can feel them growing.
77
That would be for that for blackheads and pustules or like, you know, when you get pussy cysts,
78
pimples, you want to use like a glycolic and a salicylic.
79
So glycolic has the smallest molecule of the fruit acids.
80
It goes into the pore and like liquefies what's in the pore, where salicylic acid more works like on the overall acne that's on the surface of the skin.
81
So a lot of times they'll blend the two together.
82
So using something like that, if you have problems with both or all three of those, it's almost like you need to use a couple different ingredients.
83
glycolic salicylics you can get blended and then benzoyl peroxide would be a separate
84
so you kind of need both of those ingredients
85
so that's something I would add in there is there a reason
86
that you use the rosewater toner is there a reason you use it owner are you using it too
87
and what I've heard that you need a toner yeah
88
so is it is it because you want it to clean your skin more or is it Well,
89
when I first started even having a facial routine, my mom put me on the Clinique 1 -2 -3 -step.
90
So that's what got me into, okay, toner's part of a routine.
91
And then I just heard that rose water is very good for hydrating.
92
And so I just picked that.
93
Okay.
94
So, I mean, basically a toner is a...
95
whatever product originally they made the toner
96
because they wanted to put ingredients in there
97
that strip the oils off the skin
98
because they thought it would help with acne and help with cleaning
99
so they that was like in the 90s
100
and the 80s um marketing material it was like in the 70s even is
101
that in our mom's generations is that you it toner is part of the routine you have to use a
102
And so it was like for that purpose.
103
But what they found, what they found was that that's actually stripping the oils that we need on our skin.
104
And it was actually not good for the skin because it was making your skin extra dry.
105
So they took the ingredients out of the toner, but left the toner in the market because everybody was so used to buying a toner.
106
Like you can't just take it out because if people are going to want it and they want to sell it.
107
So they just took that ingredient out.
108
So then what did it become?
109
Just basically a hydrating mist.
110
and so then it became the hydrating mist
111
so then it's like do you really need
112
that not really you could buy a product that you really need for your skin and That's just a feel -good product.
113
If you love it and you're like, I love the smell of rose and it makes me feel good when I put it on, then that's fine.
114
But it's not a necessary product to use. So there's your toner.
115
That's, like I said, you can use it or you don't have to use it.
116
You can use it for hydrating if you want to hydrate.
117
That's fine. which is good.
118
Sometimes, moisturizers can clog your skin.
119
So, the fact that you get acne, that's always one of the things I look at is what kind of moisturizer you're using.
120
Some moisturizers have things that clog pores, and if people have dry skin, it doesn't matter because they don't really get clogged pores, but when you do, you have to be careful.
121
Okay, so after toner, then you moisturize.
122
So, that's good.
123
I like that you're moisturizing.
124
The important
125
can tend to it can be caused by the moisturizer
126
so I'd like to take a look at the moisturizer
127
that people are using when they have acne to make sure the ingredients are not clogging your pores so
128
do you know what the brand is that you're using it's a ole okay um ole has a few different lines
129
switch your moisturizer.
130
Olay tends to be for an older audience, a drier audience.
131
So, I'm not sure which one you're using, but something's not working in your routine and it's not the Cetaphil or the rose water.
132
And the only other thing that you're using is Olay.
133
So, we might want to change that and see if we can get a difference, along with using things that are going to unclog your pores.
134
Acne is something that you've been dealing with for a really long time.
135
So, it's really just about under control.
136
So then you're not using an SPF, but the picking and the scarring,
137
it's important when you have acne and it's in the healing stage
138
when it's red that you're using sunscreen on it because if you're not, then the sun looks at that as a wound
139
and then your melanin comes to push forward and protect and
140
that melanin can stay there because it's a wound and it kind of damages You want to put sunscreen on,
141
especially in the healing process of acne, so that you don't get the brown spots.
142
Red goes away, brown stays, and then you have to fight the brown, which is like using things to get rid of pigmentation.
143
Also try not to pick.
144
So I feel like when you're taking care of your skin more, maybe getting some extraction facials, getting the products in there that should be clearing up the acne,
145
it'll make you not want to pick as much. as much.
146
So, you know, control the picking because the picking causes scarring.
147
And so, the other thing that I feel like you need in your routine is a sunscreen.
148
And we can get you a sunscreen that's not greasy and doesn't feel like a lot of creamy, heavy cream.
149
There's sprays, there's powders.
150
Probably the spray, I feel like for acne is good because it's really light.
151
You can mix it skin.
152
It's nice.
153
It's a physical, a lot of the sprays are physical barrier, so it doesn't go in the pores.
154
just sits on the surface so that's a really important step
155
when you have acne to keep you from getting um brown spots
156
and pigmentation after moisturizer yeah so
157
and you could pick up your cleanser i mean you're using
158
cetaphil it's a gentle cleanser it's not doing a deeper a good cleanse
159
so you know you might want to get a little uh some something a little stronger i feel that little help there.
160
But I wouldn't change that right away.
161
I'd start you on a glycolic salicylic, get you some benzoyl peroxide for the cysts, get you on a sunscreen, and then
162
get you an extraction facial so that we can really see what's going on there
163
and then start handling the acne and then caring for it later.
164
So if you want to change cleansers now, it's not a bad idea either.
165
Okay.
166
So do you use anything from a doctor's office?
167
Do you see a dermatologist?
168
No. No?
169
Okay.
170
So have you ever been on Accutane?
171
No. Do you for any reason?
172
No. On your skin?
173
Okay.
174
Have you ever had peels or microdermabrasion?
175
No. Okay.
176
Any lasers?
177
Botox?
178
No. Fillers?
179
No. Nothing?
180
All right.
181
So, do you have epilepsy, diabetes, or a pacemaker?
182
No. And do you have any metal in your body, like plates in your legs, pins in your knees?
183
Other than feelings, but no. Okay.
184
The reason we ask that is because we do something for acne called high frequency.
185
It's a current that also kills bacteria.
186
It's kind of like a electric oxygen.
187
So we do benzoyl peroxide and in the high frequency current
188
and that Helps to kill the bacteria and cysts
189
and any acne bacteria that's on the skin and
190
if you have metal in your body It can you know shock you
191
so it's important to tell people if you have metal in your body or not And then are you on any medications?
192
No, okay What?
193
The best thing to do to help you with your acne and scarring is to get you first clearing the acne, doing acne sessions that are like 30 minute sessions,
194
and then getting you on the right products.
195
And then once we heal the acne, we can start dealing with the scarring and doing things like peels and microdermabrasion.
196
You can do lasers for that, different kinds of treatments.
197
But first we want to correct the acne.
198
Like, when you eat certain foods, do you have an acne flare?
199
Or is it just kind of your skin just breaks out?
200
It's been hard to tell since I've been dealing with it since I was very young,
201
but um, one thing I did notice is that dairy was kind of triggering it a little bit,
202
but so flare up a little yeah, but even when you don't take dairy, it's still there.
203
Okay.
204
Yeah.
205
So yeah, clear that for sure all right well thank you is about it thank you
