1
I'm Giuseppe Palanzino.
2
I'm a neurosurgeon at the Mayo Clinic in Rochester, Minnesota.
3
I specialize in the open surgical and endovascular treatment of cerebrovascular disease.
4
I don't follow the Aruba recommendations.
5
I do treat a large number of patients with unraptured AVMs.
6
But I do think that the Aruba study has a value.
7
I think the Aruba study has told us and confirmed what we knew
8
that unruptured AVMs do pose a risk of bleeding over time.
9
The risk is small about 2% per year but is there
10
and real and therefore especially in young patients that risk accumulates over time.
11
The other thing that the ARUBA trial has shown us is
12
that we have to be very careful
13
when we treat a patient with a ruptured AVM because the complication rate of our treatment,
14
especially embolization, is higher than what we used to quote.
15
And the other valid point of the Aruba study is
16
that it gives us something to rely when we think
17
that an unraptured AVM should not be treated either because patient advanced age,
18
presence of comorbidity, of the perceived risk of treatment.
19
But in general I don't follow the recommendations of the Aruba trial in the sense
20
that I do recommend treatment for the majority of patients
21
that I see with unruptured arteriovenous malformations as long as I feel that the risk of the treatment strategy chosen,
22
it's acceptably low.
23
Thank you very much.
24
That's great.
