and real and therefore especially in young patients that risk accumulates over time.

The other thing that the ARUBA trial has shown us is

that we have to be very careful

when we treat a patient with a ruptured AVM because the complication rate of our treatment,

especially embolization, is higher than what we used to quote.

And the other valid point of the Aruba study is

that it gives us something to rely when we think

that an unraptured AVM should not be treated either because patient advanced age,

presence of comorbidity, of the perceived risk of treatment.

But in general I don't follow the recommendations of the Aruba trial in the sense

that I do recommend treatment for the majority of patients

that I see with unruptured arteriovenous malformations as long as I feel that the risk of the treatment strategy chosen,

it's acceptably low.
