and talk to you about super large borothrombectomy and why you ought to be including these tools into your arsenal against stroke.

I'm Sunil Sheth, I'm an associate professor at UT Health, vascular interventional neurology.

So what is super large borothrombectomy?

Here we're talking about aspiration using 88 inch diameter catheters or greater.

The whole theory behind this approach is that these are catheters that are sized to match your target vessel.

So you can see here the sizes of the different vessels

that we approach and the most common occlusion that we face in the world of LVO is M1 occlusions.

that outer diameter to the vessel much better than any of the other catheter shapes and sizes that we have.

Why is vessel matching so important.

First is this concept of flow arrest, stopping enterograde flow and preventing distal embolization.

It also, with larger inner diameter, is going to increase your aspiration force.

This means you'll have more effective thrombectomy and then with that have less likelihood of distal embolization.

So overall that translates to better first-pass reperfusion and that's really the target of every thrombectomy approach is improving that first-pass

re-canalization and in in vitro studies large-bore aspiration has shown to do that.

So as you put the catheter into the vessel you not only stop intergrade flow substantially

but once you turn on aspiration the flow reverses and this is going to prevent dislembolization and also retain the clot

in a much better fashion.

So with in vitro data we have moved on to in vivo studies

and if we look So if you look at how thrombectomy has changed over time, in the bad old days of mercy, well, it didn't really work,

but really in the last 10 years we're talking about 2015

to 2024 first pass effect has really languished in this 20 to 30 percent range where although we get bigger
24
and bigger aspiration catheters going up to the 070 range the

first pass effect has not substantially improved since the early days of stent retrievers back in 2015.

So we need a step function we need a real step up in our performance

and that's where super large has come in we want to get above at least 60% first pass effect.

We are now living in a world where we actually have randomized and prospective trial data showing that we are there.

This was one trial that was presented in the summer of 2025 from the perfuse company in the MARS trial.

This is their primary efficacy data and as you can see here first pass effect with TIKI 2C 3 or greater.

So this is not the usual 2B or greater, but 2C or greater, 61% with their millipede system.

And in the M1, again, where you have vessel matching, 77%.

These are numbers we have never seen before in rigorously collected prospective clinical trial data.

Similarly, from the Route 92 group,

the Summit Max trial was a randomized trial comparing their device against a 70 diameter catheter 74 and significantly better reperfusion results.

So just to summarize, we are now living in the world of super large-bore thrombectomy.

Super large-bore thrombectomy is the first real improvement on first-pass effect

we've ever had in the last decade since we've been doing thrombectomy.

So it's not a question of should you be doing thrombectomy, it's with super large bore catheters is
