There are many stroke scales that are currently used in the hospitals around the United States today.

The National Institute of Health Stroke Scale is the most widely used deficit rating scale in modern neurology.

Over 500,000 healthcare professionals have been certified to administer it using a web-based platform.

It is used in the hospital setting to guide therapeutic options when patient presents with ischemic stroke.

While useful, the scale is quite extensive and not pre-hospital friendly.

For the pre-hospital setting, the Cincinnati Pre-Hospital Stroke Scale was developed as a rapid way to identify strokes.

It is a scale based on the NIHSS, RAPID, but has a particular affinity towards anterior circulation strokes.

There are many other stroke scales that have been developed in order to identify the location of strokes.

The VAN score, however, as you can see, can be accounted for all the cortical areas.

The VAN score, mnemonic for verbal,

aphasia, and neglect, is a screening tool to assess for neurovascular anatomy for emergent LVO or large vessel occlusion.

It is quick and allows for early and specific emergency notifications for mobilization of appropriate teams and resources.

In order to determine whether the VAN test is positive, certain criteria must be met.

Just like the CPSS, if the patient presents with deficits in motor function, we move on to the second step.

Any deficits in vision or presence of neglect and aphasia, along with motor deficits, indicates a positive VAN score.

specificity, and accuracy in large number of patients and follows basic principles of neurological localization.

In comparison to previously published tools, the VAN tool offers simplicity,

speed, ease of use, and exclusion of mandatory scoring without sacrificing accuracy.

While regional stroke systems of care are uniquely designed,

ELVO pre-hospital assessments and bypass time windows allows for collaboration among primary and secondary stroke providers and prompt application to treatment.
