I saw a patient who was paranoid hearing voices.

She was in significant distress, not extreme, but significant, and wanted to get things off her chest, talked about her paranoid feelings,

the voices, and the person she talked to got very concerned, and she was brought involuntarily to the emergency room.

So she was quite upset about that.

She had not used any drug recently, and she did have a history of methamphetamine use, significant use apparently in the past,

two years ago about most recently.

Her urine toxicology was negative in the emergency room.

She'd been living with family, raising her son, working, attending to her hygiene,

and she was pretty well related and conversed in a pleasant way.

I spoke with two family members and they confirmed she was paranoid.

She was troubled by the paranoid feelings.

However, she was also, so to speak, functioning reasonably well, raising her son and working.

So what to do with this patient?

Did she need to go to a psychiatric hospital or not?

She had some impairment in functioning.

She was paranoid, which interfered with interacting with her family to a certain extent,

not in an extreme way.

She was, however, raising her son, working, attending to her personal hygiene.

So my overall feeling is she did not require psychiatric or involuntary psychiatric hospitalization.

She did not want voluntary psychiatric hospitalization.

Absolutely not.

She did not want to go to a hospital and she also did not want psychiatric medicine.

So I ended up recommending psychotherapy, outpatient psychotherapy, and she was agreeable to that psychotherapy to help her to cope

with what she was going through internally with the paranoid feelings, hearing the voices.

So, ultimately, it seems like this patient may have been suffering from persistent psychosis related to methamphetamine use in the past.

A very sad thing for me to think about, and it really is food for thought,

I think, regarding using drugs,

using methamphetamines specifically, in that there can apparently be ongoing long-term effects, for example, psychosis.
