Don't forget, in the US, we oftentimes have the month, First, then the day, then the year.

An insurance provider is a company that you have a contract with

who you pay monthly to help take care or pay for any medical issues you might have,

such as an accident or a doctor's appointment.

There are many different insurance providers or insurance companies out there.

There are a lot of different kinds of insurance as well.

You could have car insurance, health insurance, or homeowners insurance.

Confirmation.

This is just a statement, either written or verbal, showing that an appointment or an arrangement will occur.

For example, when you order something online,

you usually get a confirmation email to show that the item has been purchased.

A confirmation can also be to prove that something is true.

For example, in the second phone conversation, I had to confirm my birth date so that the person on the other end of the phone

A file is a folder.

with documents with information inside about something or someone.

At my doctor's office, there is a file with my name and my information on it, so that when I make an appointment or go see the doctor,
